Our policy is that deposits are due on the day of testing. We accept cash, check, Visa, Mastercard or American Express.

Your insurance company may pay all, part or none of the cost of this testing. We strongly recommend that you check with your insurance carrier prior to having the test(s) completed. Should your insurance company pay 100% of the charges, your deposit will be returned to you within a few weeks of receiving your insurance company's Explanation of Benefits.

We DO NOT accept third party liability claims (auto accident, worker compensation, etc.) A letter of authorization, in writing, signed by your claims adjuster or attorney, guaranteeing 100% payment within 30 days of billing, must be submitted to our office prior to the day of testing.

Please understand that you, the patient, are ultimately responsible for this bill.
